BACKGROUND TO THE MEDICAL PROGRAM
Zanzibar, like many regions in Africa, faces a critical shortage of healthcare workers. Contributing factors include low wages, demanding working conditions, and limited access to professional training. These challenges are intensified by the ongoing demands of HIV/AIDS treatment, which place additional strain on an already under-resourced healthcare system.
The most common causes of death in Zanzibar include malaria, HIV/AIDS, respiratory illnesses, and cardiovascular disease. While the healthcare system focuses heavily on urgent and acute care, there is also a strong emphasis on patient education and prevention to reduce long-term health risks.
Medical volunteers are placed in government hospitals and community clinics in areas such as Paje and Stone Town, offering exposure to healthcare delivery in both urban and community-based settings. This variety provides a well-rounded understanding of the local medical landscape.
This program is well suited to individuals seeking practical volunteer experience to strengthen future medical or healthcare careers. Participants may observe and assist across multiple departments, gaining broad and valuable exposure. MEDICAL VOLUNTEER PLACEMENT EXAMPLES
Mnazi Mmoja Hospital - Located in Stone Town, approximately 40 minutes from Paje, Mnazi Mmoja Hospital is the main government referral and teaching hospital in Zanzibar. It serves the wider island population and provides a range of services from general medicine to specialist care. The hospital is often busy and under-resourced, offering volunteers an authentic insight into public healthcare delivery.
Mwembeladu Maternity Hospital - A partner facility to Mnazi Mmoja Hospital, this maternity hospital handles approximately 400 births each month. It is a particularly suitable placement for midwifery students seeking exposure to antenatal care, labour, and delivery in a high-demand environment.
Volunteers may observe or rotate through departments such as General Medicine, Dental, Midwifery, Newborn Care, Paediatrics, Pharmacy, Surgery, Laboratory, Radiology, HIV Testing, Diabetes Clinic, Tuberculosis, Physiotherapy (part-time), and more. While preferences can be shared, placements are subject to availability.
On the first day, volunteers receive an introduction from the hospital matron, who discusses interests and helps coordinate department rotations. It is recommended to spend at least one week in each department to gain meaningful exposure.
This placement allows volunteers to encounter medical cases that are uncommon in many countries, including tuberculosis, typhoid, and malaria. Working in a government hospital can be challenging due to limited equipment and resources, and healthcare delivery may feel slower or less personal. Adapting to these conditions is often one of the most valuable learning aspects of the experience.
STUDENT ELECTIVE OPPORTUNITIES
Elective placements are available for students studying medicine, midwifery, or nursing. These electives are observational in nature and allow students to learn directly from local healthcare professionals. Requests for specific electives can be made during the application process.
Medical Elective - Medical students are placed in government hospitals in Stone Town or Paje, where they shadow doctors and nurses during ward rounds and observe the diagnosis and treatment of common illnesses such as malaria and typhoid.
Midwifery Elective - Midwifery students work alongside experienced midwives in maternity hospitals, observing antenatal clinics, labour, and childbirth. This placement highlights the realities and challenges of maternal care in resource-limited settings.
Nursing Elective - Nursing students shadow nurses in government hospitals, assisting with patient care, monitoring vital signs, and administering medication under supervision. Opportunities may include community outreach activities such as health checks and screenings.

OTHER THINGS TO CONSIDER AS A MEDICAL VOLUNTEER IN TANZANIA
Airport Transfers - Return airport transfers are not included. Local staff can assist with arranging onward transport once you are in-country, offering flexibility and the possibility of sharing costs with other participants. Public transport and tuk-tuks are not recommended for airport transfers due to safety and convenience concerns.
Hands-On Involvement - Levels of practical involvement depend on medical background, program length, and staff discretion. Some participants have assisted with basic tasks such as wound dressing or IV preparation, but hands-on experience is not guaranteed. Volunteers without medical training typically take observational roles.
Hospital Environment - Healthcare students who can provide proof of study may access additional learning opportunities. Volunteers should be prepared for cultural differences in patient care and emotional expression. Government hospital settings can be demanding, and volunteers are encouraged to step back and seek support if needed.
Not for Qualified Professionals - This program is designed for school leavers and students, not for fully trained professionals seeking to practise medicine abroad.
Participants Aged 17 - While open to participants aged 17 and above, younger volunteers should be confident and self-motivated. Large hospital environments can be overwhelming. Those seeking more structure may prefer alternative destinations.
Multiple Placements - In most cases, volunteers are placed at a single facility. Combining multiple placements may be possible but requires an additional local donation.
Yellow Fever Vaccination - Yellow Fever vaccination is legally required for travellers arriving from or transiting through high-risk countries. However, proof has sometimes been requested regardless. To avoid complications, obtaining the vaccination before travel is strongly recommended.
Mandatory Zanzibar Insurance - All visitors to Zanzibar must purchase local insurance through the Zanzibar Insurance Corporation (ZIC) before arrival. Failure to do so may result in denied entry.
Weekends - Project work runs Monday to Friday, leaving weekends free for rest or travel. Many volunteers use this time to explore Zanzibar’s beaches, culture, and nearby attractions.
